BAW Index revised

Hello everyone.
Attached my revised version of the BAW Index. We found the original to be missing a few important entries. I reorganized it somewhat and added sub-entries under major headings, to facilitate quick reference. I hope you'll find it useful. Please let me know if you find errors of omissions.
